RCFE
REFERRAL APPLICATION AND TERMS OF AGREEMENT
This agreement
will remain in force for a period of 1 year from today's date and will roll over
for a subsequent year unless canceled by either party by advance 30 day written
or email notice. Care Provider USA makes no guarantee of finding or
placing residents in Residential Care Facilities for the elderly (RCFEs).
Care Provider USA will be paid 100% or the corresponding referral fee listed on
direct referral eligibility fee
page of the first months resident fee (divided into to 2 consecutive monthly
payments) beginning within
working days of admission to the RCFE named on this agreement. Only the RCFE
named below is eligible for resident referral. RCFE eligibility is
non-transferable and is valid only for the RCFE license shown below. If
more than one RCFE is owned by eligible member, residents transferred or placed
in other RCFE homes will be subject to 50% of the first months fee.
The RCFE named in below in this agreement agrees to pall all collection and
court costs of any unpaid or past due fee owed to care provider USA. A
late penalty of 10% per resident daily fee will be added to accounts not paid in
full 30 days net.
